Phase 0, Step 4: Evaluation set

Spec for the developer agent. Implement exactly this. Where this spec is silent, choose the simplest option and record it in README §7 ("Step 4 deviations and additions"). The Phase 0 README constraints are binding: everything builds and tests offline, pnpm, Node 24, strict TypeScript, and Graft stores no source code. The code after step 3 is described in the repo README §5 and §7 and in step 3. Where this spec and the README disagree on an existing interface, the README wins.

The plan's Step 4 is "about 100 past agent PRs from the pilots, labeled by hand; every prompt change runs against it." The labeled PRs need pilot access and people, so they are HUMAN-TODO items. This step builds everything around them, plus a synthetic seed set that CI runs on every commit.

1. Goal

  1. Case format. A versioned on-disk format for one eval case: the CardInputs that generateCard takes, the expected labels, who labeled it and when. It is validated by a zod schema and by a committed JSON Schema generated from it.
  2. Private storage. Real cases contain pilot source code. They live in an encrypted case set outside any git working tree. They are never committed, never written to Graft's database, and are deleted in one command when a pilot leaves.
  3. Export. pnpm eval export pulls merged agent PRs from a repo through the GitHub API into unlabeled, encrypted cases. It filters by date and agent classification, samples by size and path stratum, redacts secrets, and handles rate limits.
  4. Labeling. A labeling guide (docs/phase-0/labeling-guide.md), a blind interactive pnpm eval label, double-labeling of a subset, adjudication, and Cohen's kappa.
  5. Harness. pnpm eval run runs generateCard over a case set. It uses the fake LLM by default. It calls the real Anthropic API only with --live and a key, after showing a cost estimate and getting a confirmation, with concurrency and budget caps. Live results are cached by (case, prompt fingerprint, models).
  6. Metrics and report. Confusion matrices, under-classification (floor, model, final), accuracy, out-of-scope and verifier-flag precision/recall, unavailable rate, tokens and cost. Output is JSON plus markdown, with a diff against a saved baseline and CI thresholds.
  7. Synthetic seed set. 30 fictional cases committed under eval/synthetic/, with scripted fake-LLM replies, so CI exercises the harness end to end. A regression in the floor or in combineRisk fails CI.
  8. Step 3 follow-ups from step-3 review 2: minors 1 to 3, nit 5, and the 0003 down part of nit 6 (T17).

2. Out of scope

Do not create stub code for any of these.

  • Exporting or labeling real pilot PRs (HUMAN-TODO), and the first live baseline (HUMAN-TODO).
  • Automatic scoring of the goal and behavior-change text (an LLM judge). The label stores a reference goal and key behavior changes for humans; show prints them next to the card. No metric uses them.
  • Computing the Phase 0 gate from reviewer thumbs (step 6). §10 says only how the two relate.
  • Policy evaluation or graft.yaml (step 5). The case carries its own floorConfig.
  • Message Batches, prompt tuning tools, train/test splits, and a web UI for labeling.
  • Review-2 nits 4 and 7 (shown_at comment, sweep window). They stay open for step 6.
  • Any change to github/app-manifest.json. The App already has contents, pull_requests and issues read, which is everything the export needs.

3. Decisions

# Area Choice Rationale
E1 Where the code lives src/eval/ (a tool, not a module: no handlers, no jobs) and src/bin/eval.ts. src/eval/ imports Graft code only through modules/intent/index.ts, modules/provenance/index.ts, modules/changes/index.ts, src/llm/*, src/github/client-factory.ts and src/github/errors.ts. It never imports src/db, src/queue, src/webhooks, src/http or src/config. The import rule makes "never written to Graft's DB" checkable by grep (U-BND-1). The README already plans eval/ and src/bin/eval.ts.
E2 Case unit One JSON document per PR, schemaVersion: 1. inputs is exactly CardInputs (validated with the exported cardInputsSchema). Labels are a list of label sets, one per labeler, plus an optional adjudicated set. The harness feeds inputs to generateCard unchanged, so an eval case and a production run see the same thing. Several label sets make double-labeling a property of the data, not of the tool.
E3 Schemas zod is the source of truth. eval/schema/eval-case.v1.schema.json and eval-report.v1.schema.json are generated with zod 4's z.toJSONSchema (pnpm eval schema), committed, and pinned by a test. Other tools (and humans) get a stable JSON Schema; the test stops it drifting from the code.
E4 Case sets A case set is a directory with set.json and cases/. The synthetic set is eval/synthetic/ (plaintext, committed). Each pilot has one set, $GRAFT_EVAL_DIR/<setId>/ (default GRAFT_EVAL_DIR=~/.graft-eval). The harness writes cache/ and runs/ inside the set directory. Everything about one pilot (cases, cache, reports, baseline) is under one directory, so deleting the pilot is one rm -r (E7).
E5 Privacy of pilot sets A pilot set must be outside every git working tree: the tool walks up from the resolved real path and refuses if it finds a .git entry (file or directory). Files are written with mode 0600 and directories with 0700. .gitignore also gets .graft-eval/ as a backstop. "Outside the repo" holds even if someone points GRAFT_EVAL_DIR into a checkout. The walk-up check needs no git binary.
E6 Encryption at rest Every file in a pilot set except set.json and the reports (cases, cache entries, labels) is an AES-256-GCM envelope (node:crypto, no new dependency). The key is GRAFT_EVAL_KEY: base64 of 32 random bytes (pnpm eval keygen), kept in the team password manager, never in .env in the repo. The AAD is <setId>/<relative path>, so a file cannot be moved to another set or case. set.json stores a keyId (first 8 hex characters of SHA-256 of the key) to detect a wrong key early. Synthetic sets are plaintext and must contain only fictional code. Disk encryption alone does not protect against a copy into a synced folder, a backup or a paste into a ticket. An envelope makes an accidental copy useless without the key. GCM detects tampering.
E7 Pilot leaves pnpm eval purge --set <dir> --confirm <setId> deletes the whole set directory and appends {setId, purgedAt, caseCount, by} to $GRAFT_EVAL_DIR/purge-log.jsonl (no content). HUMAN-TODO: exclude GRAFT_EVAL_DIR from backups and cloud sync, and delete any other copies. One command, one directory; the log proves deletion without keeping data.
E8 Reports carry no code Report JSON and markdown (any set) contain case ids, classes, rule ids, counts, booleans, tokens and cost. They never contain paths, titles, bodies, diff text or model prose. They are plaintext. Reports can be shared and the synthetic baseline committed. Detail is available locally through pnpm eval show.
E9 Pilot consent gate Creating a pilot set requires --consent-ref <text> (the pilot agreement reference), stored in set.json. export refuses to write into a pilot set whose consentRef is empty. Exporting pilot code needs written consent (HUMAN-TODO). The tool makes the step impossible to skip silently.
E10 Export source List closed PRs with pulls.list (state=closed, sort=updated, direction=desc, 100 per page) and stop once updated_at < from (a PR's merged_at is never after its updated_at). Keep PRs with merged_at in [from, to]. Then pulls.get and the commits (fetchPrCommits) for each candidate, and computeProvenance to classify it. Search API: not used. The search API caps at 1,000 results and 30 requests per minute. Listing is complete, and reusing step 2's classification keeps "agent PR" defined in one place.
E11 Agent filter --author agent (default) keeps PRs whose computeProvenance(...).authorKind === "agent", using --agent-logins / --agent-coauthor-emails (defaults from GRAFT_AGENT_LOGINS / GRAFT_AGENT_COAUTHOR_EMAILS, same parsing as step 2). --author bot keeps only user.type === "Bot" or a login in --agent-logins, with no commit fetch. --author any keeps every merged PR. Pilots differ: some agents add trailers, some only open PRs from a bot account.
E12 Sampling Two strata axes. Size from additions + deletions: S ≤ 50, M ≤ 300, L ≤ 1,500, XL > 1,500 (the floor's own thresholds). Paths from the file list (paths only, no patches): sensitive if computeRiskFloor reports any path rule (R1 to R8), else tests if every file is a test path, else docs if every file is a docs path, else source. Allocation: --min-per-stratum (default 2) for every non-empty stratum, the rest proportional to stratum size (largest remainder), then a seeded shuffle (--seed, mulberry32) within each stratum. A random sample of agent PRs is mostly small source changes. The rare classes (sensitive paths, huge PRs) are where under-classification hides, so each must appear. A seeded PRNG makes the sample reproducible.
E13 Two-stage fetch Stage 1 (all candidates): pulls.get, commits, pulls.listFiles paths and counts only (patches discarded at once). Stage 2 (sampled only): fetchIntentInputs (issue + files with patches), redaction, encrypted write. Candidates are capped by --max-candidates (default 400, most recently updated first). Holding patches for hundreds of PRs is pointless; refetching the sampled ones costs at most 30 pages each.
E14 Redaction Applied to the title, body, issue title and body, and every patch, line by line, never changing the line count, never touching paths or counts. secrets (always on): the patterns in §6.4, replaced by [REDACTED:<kind>]. Optional: --redact emails, --redact-host <host> (repeatable), --redact-pattern <regex> (repeatable). The case records {kind, count} per redaction kind, never the value. Removes live credentials and personal data that an eval does not need, without changing what the floor sees (paths and counts drive almost every rule).
E15 Rate limits A request wrapper (octokit.hook.wrap("request")) with an injected sleep and now: after each response, if x-ratelimit-remaining ≤ --min-remaining (default 50), sleep until x-ratelimit-reset + 1 s. On 403/429 with retry-after, or with x-ratelimit-remaining: 0, sleep and retry, at most 3 times. A wait longer than --max-wait (default 15 min) aborts with exit code 3. --max-requests (default 5,000) caps the run. Re-running the same export skips cases that already exist, so an aborted export resumes. No new dependency (@octokit/plugin-throttling is not installed). Injected time makes it testable offline.
E16 Export auth --auth app --installation-id <n> (default) uses createGitHubClientFactory with GITHUB_APP_ID, GITHUB_APP_PRIVATE_KEY_PATH and GITHUB_API_URL (default https://api.github.com). --auth token uses GRAFT_EVAL_GITHUB_TOKEN (a read-only fine-grained token). Both take fetch as an injectable for tests. Pilots will have the App installed (step 6), so the App's least-privilege token is the default. The token path covers a pilot that has not installed it yet.
E17 Gold label gold(case) = adjudicated ?? (labels non-empty and all sets equal on risk, shouldFlag and the sorted outOfScope list ? labels[0] : null). Cases with no gold are left out of metrics and counted as unlabeled or needsAdjudication. The free-text fields cannot be compared mechanically, so only the scored fields decide agreement.
E18 LLM modes --llm fake (the default): each case's fakeLlm script via scriptedLlm, or, when a case has none, the null model (writer: trivial, no out-of-scope edits; agreeingVerifier). --live: createAnthropicLlm with ANTHROPIC_API_KEY, GRAFT_LLM_MAX_RETRIES, GRAFT_LLM_REFUSAL_FALLBACK. --live is the only way to reach the network; without --live the API key is ignored. CI must never call the API. On a pilot set, fake mode measures the floor plus a model that always says trivial, which is a useful offline smoke test.
E19 Live safety Before any live call: the upper-bound cost estimate (§8.6) is printed, the run refuses if it exceeds --max-cost-usd (default 10), and it asks Continue? [y/N] on a TTY. --yes skips the question; no TTY and no --yes aborts (exit 3). During the run, a case is started only if spent + estimate(case) ≤ max-cost-usd and tokens + estimatedTokens ≤ --max-tokens (default 5,000,000); otherwise the remaining cases are skipped_budget and the report says complete: false. Concurrency --concurrency (default 4 live, 8 fake). Real money: an explicit flag, an estimate, a confirmation and two hard caps.
E20 Cache Live results only. Key: SHA-256 of [prepared.inputHash, promptFingerprint(), "live"]. inputHash already covers the inputs, PROMPT_VERSION, both models, efforts, limits and sensitive globs; the fingerprint covers prompt or schema text edited without a version bump. A result is cached when status === "ok" or the failure is not retryable. Each entry stores the CardResult, the per-model usage and the cost. --no-cache ignores reads (still writes). Fake mode never reads or writes the cache. A prompt iteration re-pays only for cases whose effective inputs changed. Transient failures are not frozen in.
E21 Cost accounting The harness wraps the client per case (meteredLlm), recording {model, usage} per call, because CardResult.usage is a sum over models. Prices live in src/eval/pricing.ts with PRICING_VERIFIED_AT; a model id matches by longest prefix (response.model may be dated). --live refuses to start if a configured model has no price. Haiku and Sonnet differ 20× in price, so a summed usage cannot be priced.
E22 Prompt tracking promptFingerprint() (new export of prompts.ts) is the SHA-256 that U-PROMPT-1 already pins: the two system prompts plus the two output schemas. Reports record PROMPT_VERSION, the fingerprint, both models and caseSetHash (§8.8). The baseline diff warns when the set hash or the models differ (not comparable) and when the fingerprint changed but the version did not. A report is only comparable to a baseline over the same cases.
E23 CI pnpm eval:ci runs the synthetic set with the fake LLM and --check against eval/baselines/synthetic.json and eval/thresholds.json. The same run is test E-SYN-1, so pnpm test covers it; CI also runs pnpm eval:ci as a separate step to exercise the CLI. Fake output is deterministic, so the whole report (metrics and per-case rows) must equal the baseline. Any floor or combining change shows up as a diff; an intended change is accepted by regenerating the baseline in the same PR.
E24 Terminal safety Everything the CLI prints from a case (title, body, issue, diff, model prose) goes through toTerminalSafe: C0 and C1 controls except \n and \t, and Unicode Cf, become their visible [U+XXXX] form. Pilot PR text is untrusted and may hold ANSI escape sequences that rewrite the terminal.
E25 Labeling is blind label shows the PR as GitHub does (title, body, issue, file list, diff). It never shows the floor, a card, or another labeler's labels. adjudicate shows the label sets, not the model output. Labels measure the model; seeing the model's answer first anchors the labeler.

The pilot block is provisional until the first live baseline (HUMAN-TODO); while provisional is true a pilot-set failure prints FAIL (provisional) and still exits 1 only with --strict. A metric whose value is null (empty denominator) passes and is listed as n/a. regressionVsBaseline gives the largest allowed worsening against the baseline (absolute, in rate units).

cases is sorted by id. skipped is null or "budget". model is null when no writer answered. flag is tp | fp | fn | tn | na. Baseline comparison ignores runId, createdAt, git, run.durationMs and run.cachedResults; everything else must be equal for matchBaseline. Fake mode prices usage with the same table (cost is then a deterministic fiction, kept so the cost code is exercised).

report.md: a header (set, run, prompt, models, mode, complete), a metric table (value; baseline; Δ), three confusion matrices, per-tag under-classification, the threshold results, and "Changed cases" (id: field baseline → current). Ids only.

7. CLI

pnpm eval <command> [flags] (tsx src/bin/eval.ts). With no command, run. Exit codes: 0 ok, 1 check failed, 2 usage or configuration error, 3 aborted (declined, budget, rate-limit wait too long). Every command that opens a pilot set needs GRAFT_EVAL_KEY.

Behavior notes:

  • export --dry-run lists strata counts and the sampled PR numbers, makes no stage-2 request and writes nothing.
  • export prints one line per stage-2 PR: exported <caseId> (<size>/<pathStratum>, redactions: <n>), and at the end the request count and the rate-limit waits. It never prints titles or paths.
  • label (interactive, node:readline/promises with injectable streams): picks the next case (sorted by id) without a label set by --as (with --double-only, only cases tagged double-label). It prints the case with show's renderer, then asks in order: risk [t/l/s]; out-of-scope files as numbers from the printed file list (1,4 or empty); goal (one line); key behavior changes (one per line, empty line ends); should-flag [y/n]; confidence [h/m/l]; notes (one line). It writes the label set, then offers the next case. q at any prompt quits without writing the current case.
  • label --apply labels.json: { "<caseId>": { risk, outOfScope, goal, keyBehaviorChanges, shouldFlag, confidence, notes } }; labeler from --as, labeledAt from the clock, guideVersion = LABELING_GUIDE_VERSION ("lg-1", a constant that must equal the version in the guide's first line, U-LBL-6). Existing set by the same labeler is replaced.
  • assign-double: tags max(min, ceil(fraction × n)) cases (seeded choice, stratified by the gold-free size/path strata recomputed from inputs) with double-label. Idempotent.
  • adjudicate: for each case whose label sets disagree (E17) and with no adjudicated, prints the case and the label sets side by side and asks the same questions. Writes adjudicated.
  • run --live without ANTHROPIC_API_KEY: exit 2. run without --live never constructs the Anthropic client. With --save-baseline the report is also written there (refused for a pilot set when the path is inside a git working tree).
  • Default --out is <set>/runs/<runId>/; for the synthetic set that directory is gitignored.

8. Metrics (definitions)

Let S be the scored cases: gold label g(i) exists and the case was not skipped. ord(trivial)=0, ord(logic)=1, ord(sensitive)=2. For each case: F(i) = risk.floor; M(i) = maxRisk(risk.writer, risk.firstPass, risk.verifier), undefined when all three are null; R(i) = risk.final. For X ∈ {F, M, R}, S_X = {i ∈ S : X(i) defined}.

  1. Under-classification rate UC_X = |{i ∈ S_X : ord(X(i)) < ord(g(i).risk)}| / |S_X|. Reported for floor, model and final (underClassificationFloor, …Model, …Final), overall and per tag. UC_R is the safety metric.
  2. Severe under-classification SUC_X = |{i ∈ S_X : g = sensitive ∧ X(i) = trivial}| / |S_X|.
  3. Over-classification OC_X = |{ord(X(i)) > ord(g)}| / |S_X|, and trivial recall TR = |{g = trivial ∧ R = trivial}| / |{g = trivial}| (the share of truly trivial PRs that could be auto-approved; feeds the 30% target).
  4. Exact accuracy ACC_X = |{X(i) = g(i).risk}| / |S_X|.
  5. Confusion matrix C_X[a][b] = |{i ∈ S_X : g = a ∧ X = b}|, rows gold, columns predicted, order trivial/logic/sensitive; per-class precision C[k][k] / Σ_a C[a][k] and recall C[k][k] / Σ_b C[k][b] (null on a zero denominator).
  6. Out-of-scope over S_ok = {i ∈ S : status = ok}: predicted P(i) = card.outOfScopeEdits[].path ∪ verifier.unexplained, gold G(i) = g(i).outOfScope. Micro precision Σ|P∩G| / Σ|P|, micro recall Σ|P∩G| / Σ|G|, and exact-set rate |{P(i) = G(i)}| / |S_ok|.
  7. Verifier flag over S_v = {i ∈ S_ok : verifier.status ∈ {agree, disagree}}: predicted flag = (status = disagree), gold g.shouldFlag. Precision TP/(TP+FP), recall TP/(TP+FN). verifierErrorRate = |{status = error}| / |S_ok|.
  8. Unavailable rate |{status = unavailable}| / |run cases| (run cases = not skipped, gold or not), with counts per failureReason.
  9. Injection detection over cases tagged injection: |{instructionsInData ∨ "injection_marker" ∈ floorRules}| / |tagged|.
  10. Card acceptable (offline proxy for a thumbs-up) over S_ok: R = g.risk ∧ P = G ∧ (flag correct or na). Reported as cardAcceptableRate; see §10.
  11. Tokens and cost: sums of the four usage counters and calls over run cases; costUsd = Σ_calls Σ_counter tokens × price(model, counter) / 1e6; also costUsdThisRun (non-cached results only) and costPerCaseUsd.
  12. Upper-bound estimate for a case (E19): out = MAX_OUTPUT_TOKENS × (floor = trivial ? 3 : 2), in = estimatedTokens − out, usd = (in × price(strong).input + out × price(strong).output) / 1e6. It ignores caching and the cheaper fast model, so it is an upper bound.
  13. caseSetHash = SHA-256 of the sorted list of caseHash(c) + ":" + goldDigest(c), where caseHash is SHA-256 of JSON.stringify([c.id, c.inputs, c.floorConfig]) and goldDigest is SHA-256 of the JSON of {risk, shouldFlag, outOfScope} of the gold (or "none").

Rates are numbers in [0, 1] rounded to 4 decimals; an empty denominator gives null.

8.1 Agreement (agreement)

For each pair of labelers with at least 5 shared cases (using each labeler's own set, not the adjudication): - Risk: Cohen's kappa κ = (p_o − p_e) / (1 − p_e), p_o = share of shared cases with equal labels, p_e = Σ_k p_A(k) · p_B(k). When p_e = 1, κ = 1 if p_o = 1, else null. Also linear-weighted kappa with weights w_ab = |ord(a) − ord(b)| / 2: κ_w = 1 − Σ w·o / Σ w·e (observed and chance proportions). - Should-flag: Cohen's kappa over {true, false}. - Out-of-scope: Cohen's kappa over per-file decisions (every file of every shared case, in / out). - Output: n, raw agreement and kappa for each, plus the list of disagreeing case ids. Target (labeling guide): risk κ ≥ 0.6; below it, revise the guide and relabel before trusting metrics.

9. Synthetic seed set

All cases use the fictional repository fictional-org/shop-demo, PR numbers 101 to 130, fictional names, no real secrets (a fake token in a case is built so it does not match §6.4 unless the case is about redaction; none is in this set). labels has one set by graft-synthetic, guideVersion: "lg-1". verifier: "agree" means agreeingVerifier (which answers risk_class: "trivial"). "fast"/"strong" says which writer the floor selects.

Expected headline values (fake mode): UC_R = 0, SUC_R = 0, UC_F = 6/30 (003, 022, 023, 026, 028, 030), SUC_F = 1/30 (026), ACC_R = 28/30 (027 and 029 over-classified), out-of-scope precision and recall 1, flag precision and recall 1, unavailable rate 2/30. If a path or phrase does not trigger the floor stated in the table, change the case (path or wording), never a floor rule, and note it in README §7. Every floor rule id appears in at least one case's floorRules (U-SYN-5).

10. Relation to the Phase 0 gate

The gate is online: on pilot PRs, the share of effective reviewer votes that are 👍 (feedbackSummary, PR authors excluded) must reach 90% (step 6). The eval set is offline and does two different jobs. (1) Safety: UC_final and severe under-classification have no online equivalent (a reviewer rarely downvotes a card for being too relaxed), so they are hard gates on every prompt or floor change. (2) Prediction: cardAcceptableRate (§8.10) is a proxy for a thumbs-up. Before step 6 computes the gate, run the live pilot set; a cardAcceptableRate well below 0.9 means the gate will fail and the prompts need work first. Step 6 calibrates the proxy by comparing it with the thumbs on PRs that are in both.

11. Tasks

Each task lists its acceptance criteria (AC). Test IDs are in §12.

T1. Exports and scripts

  • Add the §5.1 exports; promptFingerprint() in prompts.ts.
  • package.json: "eval": "tsx src/bin/eval.ts", "eval:ci": "tsx src/bin/eval.ts run --set eval/synthetic --baseline eval/baselines/synthetic.json --check".
  • .gitignore: .graft-eval/, eval/synthetic/cache/, eval/synthetic/runs/.
  • CI: add - run: pnpm eval:ci after pnpm test.
  • AC: U-PROMPT-1 extended; grep -rn "from \"\.\./\.\./modules/[a-z]*/[a-z-]*\.js\"" src/eval finds only index.js imports (U-BND-1).

T2. Case schema (case-schema.ts, report-schema.ts)

  • §6.1 schema with the cross-field refinements (pilot vs synthetic, oos paths in the file list, unique labelers, sorted unique outOfScope, id rules). gold(), caseHash().
  • pnpm eval schema --write writes both JSON Schemas (z.toJSONSchema, draft 2020-12, $id https://graft.invalid/schema/eval-case.v1.json). Refinements that JSON Schema cannot express are listed in the schema's top-level description.
  • AC: U-CASE-1 to -9.

T3. Crypto and store (crypto.ts, store.ts)

  • §6.3 envelope; assertPrivateLocation (E5); atomic write (temp file in the same directory, fsync, rename), modes 0600/0700; writeCase refuses a plaintext write for a pilot set and a sealed write for a synthetic set; openSet checks keyId; readCase refuses a .case.json inside a pilot set and a .case.enc inside a synthetic set.
  • purgeSet: refuses kind: synthetic, a confirm that differs from setId, and a directory without set.json; removes the directory; appends to <evalRoot>/purge-log.jsonl.
  • AC: U-CRYPT-1 to -5, U-STORE-1 to -9.

T4. Terminal renderer (terminal.ts)

  • toTerminalSafe (E24). renderCase(c, { withResult?: CardResult }): header (id, repo, PR, merged, author kind), title, body, issue, numbered file list (n. status path (+a −d)), then each patch under a --- path line. With a result: goal, behavior changes, out-of-scope, risk (final/floor/writer/verifier), verifier status, then the gold label if any.
  • AC: U-TERM-1, -2.

T5. Redaction (redact.ts)

  • redactInputs(inputs, opts) → { inputs, redactions } per §6.4.
  • AC: U-RED-1 to -6.

T6. Sampling (sample.ts)

  • sizeStratum, pathStratum(files, floorConfig), allocate(strata counts, n, minPer), sample(candidates, o); mulberry32 seeded; candidates sorted by (repository, prNumber) before sampling.
  • AC: U-SAMP-1 to -5.

T7. Rate limiting (rate-limit.ts)

  • installRateLimit(octokit, { sleep, now, minRemaining, maxWaitMs, maxRequests, onWait }) per E15. Throws EvalAbort("rate_limit_wait" | "max_requests").
  • AC: U-RL-1 to -5.

T8. Fake GitHub extensions (test/helpers/fake-github-api.ts, github-builders.ts)

  • GET /repos/{o}/{r}/pulls with state, sort=updated, direction=desc, per_page, page, and a Link: rel="next" header; FakePull gains updated_at, merged_at, user if missing.
  • state.rateLimit = { limit, remaining, reset }: every response carries x-ratelimit-*, remaining decrements per request; when 0, responses are 403 API rate limit exceeded until the injected clock passes reset.
  • Existing failures facility used for 429 with retry-after.
  • AC: existing tests unchanged and green.

T9. Export (export.ts)

  • Pipeline per E10 to E16: list → date filter → existing-case skip → stage 1 (classification and strata) → sample → stage 2 (fetchIntentInputs with taskIssueNumber from the Graft-Task #N value, redaction, case build, writeCase). defaultFloorConfig copied to each case. Case id <setId>--<repo name>-<prNumber>.
  • Errors: a 404/410 on one PR skips it (counted); other GitHub errors abort with exit 2 after writing what was done.
  • AC: U-EXP-1 to -10.

T10. Labeling (label.ts)

  • Interactive and --apply (§7), assign-double, adjudicate. Blind (E25). LABELING_GUIDE_VERSION = "lg-1".
  • AC: U-LBL-1 to -7.

T11. Kappa (kappa.ts) and agreement

  • §8.1.
  • AC: U-KAPPA-1 to -5.

T12. Fake models, metering, pricing (fake-models.ts, metered.ts, pricing.ts)

  • llmForCase(c): scriptedLlm from fakeLlm ("agree" → agreeingVerifier), else the null model.
  • Prices (USD per million tokens, PRICING_VERIFIED_AT = "2026-10-06"): claude-haiku-5-5 input 0.10, output 0.50, cache read 0.01, cache write 0.125; claude-sonnet-5-5 input 2.00, output 10.00, cache read 0.20, cache write 2.50. Haiku's higher rate above 100K prompt tokens is not modelled (Graft prompts stay below 60K: 120,000 diff characters plus 28,000 for text). Unknown model → null cost.
  • AC: U-PRICE-1, -2; U-HAR-3.

T13. Harness and cache (harness.ts, cache.ts)

  • runEval per §5.2 and E18 to E21: load and validate every case (a schema error aborts with the case id), compute gold, prepare each case (prepareCardInputs) for the estimate and the cache key, look up the cache, estimate, confirm, run with a promise pool, write cache entries, build rows, compute metrics, build the report. GenerateOptions: models, floor = the case's floorConfig, timeoutMs (GRAFT_LLM_TIMEOUT_MS, default 120000), deadlineMs 600000, nowMs, no log.
  • One case's unexpected exception (a bug, not an LLM failure) aborts the run with the case id; LLM failures are results (step 3 contract).
  • AC: U-HAR-1 to -10.

T14. Metrics and report (metrics.ts, report.ts)

  • §8 and §6.6; diffReports, renderMarkdown, checkThresholds. Pilot sets read <set>/baseline.json by default; synthetic reads eval/baselines/synthetic.json.
  • AC: U-MET-1 to -10, U-RPT-1 to -6.

T15. CLI (src/bin/eval.ts, env.ts)

  • §7 commands; env.ts parses GRAFT_EVAL_DIR, GRAFT_EVAL_KEY, GRAFT_EVAL_GITHUB_TOKEN, ANTHROPIC_API_KEY, GRAFT_LLM_MODEL_FAST/STRONG, GRAFT_LLM_TIMEOUT_MS, GRAFT_LLM_MAX_RETRIES, GRAFT_LLM_REFUSAL_FALLBACK, GRAFT_AGENT_LOGINS, GRAFT_AGENT_COAUTHOR_EMAILS, GITHUB_APP_ID, GITHUB_APP_PRIVATE_KEY_PATH, GITHUB_API_URL, with the same defaults and parsing as src/config (copy the parsers; do not import src/config). The CLI entry exports main(argv, deps) with injectable env, stdin, stdout, isTTY, fetch, now, sleep, llmFactory; src/bin/eval.ts calls it with the real ones.
  • Output never contains prose or code from a pilot case except in show and label/adjudicate.
  • AC: U-CLI-1 to -8.

T16. Synthetic set, baseline, labeling guide, docs

  • The 30 cases of §9, set.json, eval/synthetic/README.md; eval/thresholds.json (§6.5); pnpm eval:ci --save-baseline eval/baselines/synthetic.json once, committed.
  • docs/phase-0/labeling-guide.md with the content in §13.
  • README: §5 layout (src/eval), a new section "Evaluation set" (commands, where data lives, privacy, how to update the baseline), §7 "Step 4 deviations and additions". .env.example: commented GRAFT_EVAL_DIR, GRAFT_EVAL_KEY ("never here: password manager"), GRAFT_EVAL_GITHUB_TOKEN. HUMAN-TODO: §14.
  • AC: U-SYN-1 to -6, E-SYN-1, E-SYN-2.

T17. Step 3 follow-ups (review 2)

  1. Retry key (minor 1). Replace the :{deliveryId} suffix with a 5-minute bucket: intent-retry:{changeId}:{headSha}:{inputHash16}:{attempt}:{floor(nowMs / 300000)}, nowMs from ctx.clock. A pending-only singleton is not available on our standard queues (README §7, pg-boss 12), hence the bucket. Update the comment above the send and README §7. AC: I-INT-22 (two triggers with the same inputs within one bucket → one retry job; a trigger in the next bucket after the first chain completed → a new retry job; I-INT-21 still passes).
  2. The D11 note is a display (minor 2). Migration 0004_intent_display_pending: intent_displays.status CHECK becomes ('ok','unavailable','pending') (drop the auto-named intent_displays_status_check and add it back with the new list; down restores the old one after deleting pending rows). src/db/schema.ts type updated. Inside the note's lock, after the comment write, insert a display (change_id, card_id = previous card id, status 'pending', shown_at = ctx.clock.now()). Reaction attribution treats pending like unavailable (dropped). latestPublishedCard skips pending displays (it must return the last real card). markPublished keeps comparing with the last display of any status, so the next ok or unavailable publish after a note always adds a row. AC: I-FB-10 (a reaction made while the note is shown is dropped; one made after the new card is shown is credited to the new card), I-DB-9 (0004 up/down).
  3. Display gap on a failed publish (minor 3). Inside the publish lock, write the check run first, then the comment, then markPublished with now taken right after the comment write. A failed check-run call therefore leaves the old comment and no new display; a failed comment call leaves no new display either (the check run's buttons attribute exactly through external_id). Update the order note in README §7. AC: I-INT-23 (fail PATCH/POST check-runs once → no display row and the comment unchanged; the pg-boss retry publishes once and records exactly one display), I-E2E-3 still passes.
  4. 0003 down (nit 6). DROP TABLE IF EXISTS intent_displays and intent_spend. AC: I-DB-9 also rolls back 0003 on a database where the two tables were dropped by hand.
  5. escapeProse dead line (nit 5). Keep the <!-- rewrite in escapeProse and add the comment: "Unreachable: < and ! are already escaped above; kept for defence in depth. The mutation check for this rule applies to codeSpan." No behaviour change.

12. Test plan

All tests run offline. Use the IDs as it(...) title prefixes. GitHub is reached only through createFakeGitHubApi().fetch; the LLM only through scriptedLlm/createFakeLlm/forbiddenLlm or createAnthropicLlm with an injected fetch. The clock and sleep are injected. File tests use fs.mkdtemp under os.tmpdir() (outside the repo) and, for "inside a git tree" cases, a temp dir containing an empty .git directory. Test keys are generated in the test (generateKey()), never committed.

Unit (test/unit/eval-*.test.ts)

ID Case
U-BND-1 No file in src/eval/ or src/bin/eval.ts imports src/db, src/queue, src/webhooks, src/http, src/config, or a module file other than index.ts (grep over import lines).
U-BND-2 No console.* or logger call in src/eval/ mentions title, body, patch, goal, behaviorChanges or reason (log-hygiene style grep), except in terminal.ts.
U-CASE-1 Every synthetic case parses with evalCaseSchema; the §6.1 example parses.
U-CASE-2 Pilot case with non-null fakeLlm → rejected; synthetic id without syn- → rejected; pilot id without <setId>-- → rejected.
U-CASE-3 outOfScope path not in inputs.files → rejected; unsorted or duplicate → rejected.
U-CASE-4 Two label sets by the same labeler → rejected; labeler a@b.c → rejected.
U-CASE-5 Pilot source.repository ≠ inputs.repositoryFullName → rejected.
U-CASE-6 Committed eval/schema/*.json equal z.toJSONSchema output byte for byte (after JSON.stringify(…, null, 2) + "\n").
U-CASE-7 gold: adjudicated wins; two agreeing sets (different goal text) → first; disagreeing risk / flag / oos without adjudication → null; no labels → null.
U-CASE-8 caseHash ignores labels and tags; changes with one patch character or a glob.
U-CASE-9 ajv (2020-12) validates all synthetic cases against the committed case schema, and a sample report against the report schema.
U-CRYPT-1 seal then open round-trips (including non-ASCII); two seals of the same text differ (IV).
U-CRYPT-2 Wrong key, flipped ct byte, flipped tag byte, different AAD → EvalCryptoError; the message contains no plaintext.
U-CRYPT-3 parseKey rejects 31 and 33 bytes and non-base64.
U-CRYPT-4 keyId is stable and 8 hex chars.
U-CRYPT-5 A sealed case file contains none of the case's title, body or patch substrings.
U-STORE-1 createSet for a pilot inside a dir with .git (any ancestor) → EvalLocationError; outside → ok; modes 0700 dir / 0600 files.
U-STORE-2 Pilot createSet without consentRef → error.
U-STORE-3 writeCase in a pilot set writes only cases/<id>.case.enc; in a synthetic set only .case.json.
U-STORE-4 readCase refuses a planted .case.json in a pilot set and a .case.enc in a synthetic set.
U-STORE-5 openSet with a key whose keyId differs → "wrong GRAFT_EVAL_KEY" before any decryption.
U-STORE-6 A case file moved from set A to set B (same key) fails to open (AAD).
U-STORE-7 Atomic write: a failing rename (injected) leaves the old file intact and no temp file.
U-STORE-8 purgeSet: wrong confirm → refused, nothing deleted; synthetic → refused; ok → directory gone, one purge-log.jsonl line with setId, caseCount, no paths.
U-STORE-9 Cache entries of a pilot set are sealed (.json.enc) and contain no plaintext goal.
U-TERM-1 toTerminalSafe("\u001b[2J\u009b31m‮ok") contains no ESC, no C1, no U+202E, and shows [U+001B], [U+009B], [U+202E]; \n and \t kept.
U-TERM-2 renderCase numbers files from 1 in inputs.files order and never prints the floor or a card unless withResult is given.
U-RED-1 Each §6.4 secret kind is replaced; {kind, count} reported; the value appears nowhere in the output.
U-RED-2 A patch keeps its line count and its +/-/space prefixes after redaction, including a multi-line private key.
U-RED-3 assignment keeps API_KEY = and replaces only the value; values shorter than 8 chars untouched.
U-RED-4 emails off by default; on: replaced, noreply@anthropic.com kept.
U-RED-5 Paths, statuses and counts are byte-identical before and after; computeRiskFloor path reasons identical.
U-RED-6 --redact-host corp.example replaces api.corp.example and corp.example, not notcorp.example.
U-SAMP-1 sizeStratum boundaries: 50 → S, 51 → M, 300 → M, 301 → L, 1500 → L, 1501 → XL.
U-SAMP-2 pathStratum: .github/workflows/x.yml → sensitive; only *.test.ts → tests; only docs/** → docs; mixed source → source; configured glob → sensitive.
U-SAMP-3 allocate gives every non-empty stratum at least min, sums to n, is proportional otherwise (largest remainder), and never exceeds a stratum's size.
U-SAMP-4 Same seed and candidates in any input order → same sample; another seed → different.
U-SAMP-5 n ≥ candidates → all candidates.
U-RL-1 Response with remaining: 10, minRemaining: 50, reset in 120 s → sleep(121000) before the next request.
U-RL-2 429 retry-after: 3 → sleeps 3000 ms and retries; succeeds on the second try.
U-RL-3 403 with remaining: 0 and a reset 30 s ahead, four times in a row → three sleeps and retries, then the fourth 403 propagates (classified rate_limited).
U-RL-4 Required wait > maxWait → EvalAbort("rate_limit_wait") without sleeping.
U-RL-5 maxRequests: 3 → the fourth request throws EvalAbort("max_requests").
U-EXP-1 7 closed PRs over 2 pages (Link header), 4 merged in range, 1 merged before, 1 not merged, 1 merged after → stage 1 sees exactly the 4; listing stops at the first PR with updated_at < from.
U-EXP-2 --author agent: PR with a Graft-Agent trailer kept (authorKindReason: "trailer"), human PR dropped, bot author kept. --author bot makes no commit request.
U-EXP-3 Stage 2 only for sampled PRs (request log); stage 1 discards patches (no patch string retained, checked through a spy on the case builder).
U-EXP-4 Written case: sealed, schema-valid, labels: [], source fields from the API, defaultFloorConfig copied, taskIssueNumber used for the linked issue when the body has no closing keyword.
U-EXP-5 Re-running the same export writes nothing new and makes no stage-2 request for existing ids.
U-EXP-6 Pilot set with empty consentRef (hand-edited) → refused before any request.
U-EXP-7 A 404 on pulls.get for one PR → skipped and counted; others exported.
U-EXP-8 Fake rate limit at 0 with reset ahead → the injected sleep is called and the export completes.
U-EXP-9 --dry-run writes nothing and makes no pulls/{n}/files request with patches needed (stage 1 only).
U-EXP-10 Redaction applied: a fake token in a PR body is absent from the decrypted case; source.redactions has the count.
U-LBL-1 --apply writes a label set with labeler, labeledAt from the clock, guideVersion: "lg-1"; replaces an existing set by the same labeler.
U-LBL-2 Interactive with scripted stdin (l, 2, goal, 2 changes, empty, y, h, notes) → the expected label set; the oos number maps to the right path.
U-LBL-3 Interactive output (captured stdout) contains no floor rule id, no risk_class and no other labeler's label (blind).
U-LBL-4 q mid-case writes nothing for that case.
U-LBL-5 assign-double --fraction 0.2 --min 3 on 10 cases tags 3; second run changes nothing.
U-LBL-6 LABELING_GUIDE_VERSION equals the version on the first line of docs/phase-0/labeling-guide.md.
U-LBL-7 adjudicate lists only cases with disagreeing sets and no adjudicated; writes adjudicated; gold then returns it.
U-KAPPA-1 Identical label lists → κ = 1.
U-KAPPA-2 Textbook example: A/B over 50 binary items with counts (20, 5, 10, 15) → κ = 0.4 (± 1e-9).
U-KAPPA-3 p_e = 1 and p_o = 1 → 1; all one category for A and another for B → κ ≤ 0.
U-KAPPA-4 Weighted kappa: trivial/sensitive disagreements weigh twice trivial/logic ones (hand-computed fixture).
U-KAPPA-5 agreement output: pairs with < 5 shared cases omitted; oos kappa over per-file decisions; disagreeing ids listed.
U-PRICE-1 cost for a usage of 1M each counter on Sonnet = 2 + 10 + 0.20 + 2.50.
U-PRICE-2 Dated id claude-sonnet-5-5-20260901 priced as Sonnet; unknown model → null.
U-MET-1 UC/SUC/OC/ACC on a hand-built 6-row fixture for floor, model and final (values in the test).
U-MET-2 Model risk undefined (unavailable) → row left out of S_M but in S_R.
U-MET-3 Confusion matrix and per-class precision/recall; zero column → null.
U-MET-4 OOS micro precision/recall, with the verifier's unexplained counted as predicted; non-ok rows excluded.
U-MET-5 Flag metrics exclude error and unavailable; tp/fp/fn/tn counted.
U-MET-6 Unavailable rate counts ungolded cases; skipped cases excluded.
U-MET-7 Injection detection over injection-tagged rows.
U-MET-8 cardAcceptableRate on a fixture.
U-MET-9 Empty input → every rate null, counts 0.
U-MET-10 caseSetHash changes when a gold risk changes, not when a goal text or a tag changes.
U-RPT-1 diffReports on identical reports differing only in runId, createdAt, git, durationMs, cachedResults → equal.
U-RPT-2 One case's final changed → diff lists it and matchBaseline fails.
U-RPT-3 Different caseSetHash or models → diff marked "not comparable"; fingerprint changed with the same PROMPT_VERSION → warning.
U-RPT-4 checkThresholds: max/min, null → pass with n/a, regressionVsBaseline, provisional pilot block without --strict → not fatal.
U-RPT-5 renderMarkdown contains the metric table and changed-case ids, and none of the paths or titles from the fixture cases.
U-RPT-6 A pilot-set report JSON contains no string that occurs as a path, title, body or goal in its cases (scan of every string value).
U-HAR-1 Fake mode never calls llmFactory and never touches cache/.
U-HAR-2 Live mode with an injected fake client: confirm called once with the estimate; declined → aborted: "declined", no LLM call.
U-HAR-3 Estimate over maxCostUsd → aborted: "over_budget" before confirm.
U-HAR-4 Budget during run: maxCostUsd allows 2 of 5 cases (concurrency 1) → 3 skipped: "budget", complete: false.
U-HAR-5 Cache: second live run makes zero LLM calls and reports cached: true, costUsdThisRun: 0; editing one patch re-runs exactly that case; --no-cache re-runs all.
U-HAR-6 Retryable unavailable results (fake rate_limited) are not cached; too_large is.
U-HAR-7 Concurrency: with a gated fake, never more than concurrency cases in flight.
U-HAR-8 Per-model metering: a case with fast writer, strong escalation and verifier records three calls with their models; cost uses each model's price.
U-HAR-9 A case that fails schema validation aborts with its id before any call.
U-HAR-10 Case floorConfig reaches the floor: syn-011 without its glob would be trivial (control run with the glob removed in memory).
U-CLI-1 run --live without ANTHROPIC_API_KEY → exit 2; run without --live with a key set → llmFactory not called.
U-CLI-2 run --live on a non-TTY without --yes → exit 3, no call.
U-CLI-3 run --check on the synthetic set → exit 0; with a hand-modified baseline → exit 1 and the diff printed.
U-CLI-4 Opening a pilot set without GRAFT_EVAL_KEY → exit 2 with a clear message.
U-CLI-5 purge without --confirm → exit 2.
U-CLI-6 keygen prints 44 base64 chars decoding to 32 bytes.
U-CLI-7 export stdout contains case ids and counts, never a PR title from the fake.
U-CLI-8 Unknown command or flag → exit 2 with usage.
U-SYN-1 30 cases, ids syn-001…syn-030 unique and matching file names, all kind: synthetic, all with exactly one label set by graft-synthetic.
U-SYN-2 No synthetic case matches any §6.4 secret pattern (the redactor reports zero).
U-SYN-3 repositoryFullName is fictional-org/shop-demo everywhere.
U-SYN-4 Every case whose script has no failure yields status: ok and verifier status ≠ error (scripts are valid model output).
U-SYN-5 The union of floorRules over the synthetic run covers all 15 FloorRuleId values.
U-SYN-6 Each case's floor and final equal the §9 table (table encoded in the test).
E-SYN-1 runEval over eval/synthetic in fake mode: report equals eval/baselines/synthetic.json (ignoring the §6.6 fields) and every synthetic threshold passes; UC_R = 0.
E-SYN-2 The synthetic run reproduces the §9 headline values exactly: UC_R = 0, SUC_R = 0, UC_F = 6/30, SUC_F = 1/30, ACC_R = 28/30, out-of-scope and flag precision/recall 1, unavailable rate 2/30.

Integration (Postgres; T17)

ID Case
I-INT-22 Retry bucket: two intent.generate runs (different deliveries, same inputs) fail retryably within the same 5-minute bucket → one retry job; after that chain completes, a failure in the next bucket queues a new retry job.
I-INT-23 Check-run publish fails once: no intent_displays row and the comment body unchanged; the pg-boss retry publishes, and exactly one display row exists afterwards.
I-FB-10 Pending note shown at t1, new card at t2; a 👍 at t1 < t < t2 is dropped; a 👍 after t2 is credited to the new card.
I-DB-9 0004 up/down round trip; 0003 down succeeds on a database where intent_displays and intent_spend were dropped by hand.

13. Labeling guide content (docs/phase-0/labeling-guide.md)

First line: Labeling guide lg-1. Sections, each short, with fictional examples:

  1. Purpose and rules. Label what the diff does, not what the description says. Label blind: do not open the PR on GitHub, Graft's comment or the check. Budget 5 to 10 minutes a case; if a case needs more, label it with confidence: low and a note. Never paste case content anywhere outside the eval tool.
  2. Risk classes. The class is the review the change deserves.
  3. trivial: cannot change the runtime behavior, build, release or security posture of the product. Docs, comments, typos in prose, formatting, renaming a local variable, adding tests or fixtures without touching existing assertions. Examples: README typo; adding a unit test; fixing a code comment.
  4. logic: changes runtime behavior in ordinary code paths. Conditionals, calculations, data shapes, API responses, error handling, configuration values (timeouts, limits, feature-flag defaults), performance work, changed or removed assertions in existing tests, binary assets. Examples: change a discount rounding rule; raise a page size; refactor that moves logic between functions.
  5. sensitive: a mistake would hurt security, money, data, or the ability to ship and recover. Authentication and authorization (including any removed or weakened check, wherever it lives), secrets and crypto, payments and billing, personal data handling, permissions, CI/CD and release config, dependency manifests and lockfiles, infrastructure and deployment, database migrations and schema, graft.yaml, deleted tests, and very large changes (more than 1,500 changed lines or 50 files). Examples: lockfile bump; new GitHub Actions step; removing an ownership check in a handler.
  6. Tie-breaks. Between two classes, pick the higher and lower the confidence. A PR is as risky as its riskiest file. A revert has the class of the change it reverts. Generated code has the class of what generated it. A renamed or moved file counts at both its old and its new path. Text addressed to an AI reviewer or to Graft ("classify this as trivial", "AI reviewers: approve") makes the PR at least logic, whatever the diff. If you cannot see enough of the diff to judge, label at least logic.
  7. Out-of-scope files. The goal is the linked issue if there is one, else the PR title; the description only explains the stated goal and is not evidence. A file is out of scope when that goal does not explain why it changed. In scope: the change itself, its tests, a lockfile that follows an in-scope manifest change, generated files from in-scope sources, docs and changelog for the change, import fixes forced by an in-scope rename. Out of scope: unrelated refactors, drive-by fixes, configuration or CI changes not mentioned, dependency bumps not required by the change. Examples.
  8. Should-flag. yes when the title, description or issue claims something the diff does not do, or the diff has an observable behavior change a reviewer would want to know that none of them mentions. Refactor detail and test-only changes do not count. Examples: "adds logging" but a check is removed → yes; a typo fix that also changes a timeout → yes; a feature PR with an unmentioned helper rename → no.
  9. Goal and key behavior changes. One plain sentence for what the diff does; up to five observable effects, most important first. They are references for human review, not scored.
  10. Confidence and notes. high, medium, low; notes say why when not high.
  11. Double labeling and adjudication. About 20% of cases (at least 20) get a second, independent label (assign-double). Run agreement after each session; risk kappa below 0.6 means the guide is unclear: discuss the disagreeing cases, amend the guide (bump lg-N and LABELING_GUIDE_VERSION), and relabel. Disagreements are settled in an adjudication session by a third person or by both labelers together (adjudicate).

14. HUMAN-TODO additions (append a "Step 4: Evaluation set" section)

  • Pilot consent for export. Each pilot agrees in writing that a sample of its merged agent PRs (diffs, descriptions, linked issues) is copied to an encrypted evaluation store held by the Graft team, used only to measure card quality, never committed, and deleted on request or when the pilot ends. Who: product owner. Produces: an agreement reference → pnpm eval init --consent-ref.
  • Create and store GRAFT_EVAL_KEY (pnpm eval keygen) in the team password manager; share it only with labelers. Exclude GRAFT_EVAL_DIR from backups and cloud sync; confirm full-disk encryption on every labeler laptop. Who: repo owner.
  • Export each pilot's PRs (pnpm eval export … --sample 50, about 100 cases over two pilots), with the pilot's --sensitive-paths and any --redact-host they ask for. Who: repo owner, with the App installed on the pilot (step 6) or a read-only token.
  • Labeling sessions. Two labelers read docs/phase-0/labeling-guide.md, label every case, run assign-double and double-label the subset, run agreement (target risk κ ≥ 0.6), adjudicate. Who: two engineers familiar with the pilot code. Produces: gold labels for about 100 cases.
  • First live baseline. pnpm eval run --set <pilot set> --live (check the printed estimate; set --max-cost-usd), review report.md, then --save-baseline <set>/baseline.json. Set the pilot thresholds in eval/thresholds.json from it and set provisional: false. Record the date, models, PROMPT_VERSION and cost in docs/plan/development-plan.md. Who: repo owner.
  • Check prices in src/eval/pricing.ts against Anthropic's pricing page before the first live run and update PRICING_VERIFIED_AT. Who: repo owner.
  • When a pilot leaves: pnpm eval purge --set <dir> --confirm <setId> on every labeler machine, and delete any other copies. Who: repo owner.

15. Definition of done

From a clean clone with Docker running and no network beyond install and the image pull:

pnpm install --frozen-lockfile
pnpm db:up
pnpm lint
pnpm typecheck
pnpm test # every §12 ID present and passing; steps 1–3 IDs still passing
pnpm build
pnpm eval:ci # exit 0; prints the synthetic report summary
pnpm eval schema && git diff --exit-code eval/schema/ # committed schemas are current
pnpm eval validate --set eval/synthetic # 30 cases, 30 gold
pnpm db:migrate && pnpm db:status                          # 0001–0004 applied
pnpm db:rollback && pnpm db:status && pnpm db:migrate      # only 0004 rolled back, then re-applied
grep -rnE "@ts-(ignore|expect-error)" src/ && exit 1 || true
grep -rnE "from \"\.\./(db|queue|webhooks|http|config)/" src/eval/ && exit 1 || true
grep -rn "U-EXP-\|U-HAR-\|U-MET-\|E-SYN-\|I-INT-22\|I-FB-10" test/ | wc -l # non-zero

Manual check (record it in the PR description; no network needed): 1. GRAFT_EVAL_KEY=$(pnpm -s eval keygen) GRAFT_EVAL_DIR=$(mktemp -d) pnpm eval init --set $GRAFT_EVAL_DIR/pilot-demo --kind pilot --consent-ref test → ok; the same with --set ./tmp-pilot inside the repo → exit 2 (inside a git working tree). 2. pnpm eval run --set eval/synthetic --live with no key → exit 2; with ANTHROPIC_API_KEY=dummy and stdin not a TTY → exit 3 after printing the estimate, and no request leaves the machine. 3. pnpm eval show --set eval/synthetic --case syn-019-injection-body prints the case with the injection text visible and no escape sequences.

Mutation checks (the reviewer reverts each and sees the named tests fail): - combineRisk's floor term → E-SYN-1 (UC_R > 0), plus the step 3 tests. - Removing .github/workflows/** from CI_CONFIG_GLOBS → E-SYN-1, U-SYN-6. - writeCase writing plaintext for pilot sets → U-STORE-3, U-CRYPT-5. - Dropping the AAD → U-STORE-6. - assertPrivateLocation returning early → U-STORE-1. - Counting the verifier's unexplained out of the predicted out-of-scope set → U-MET-4, E-SYN-1. - T17.3 order reverted (comment before check run) → I-INT-23. - T17.2 pending display not written → I-FB-10.
